The Health Care Improvement Foundation wants to hear from Montgomery County youth about what you think people need to be healthy and safe. They want our local youth to tell us about the significant health experiences and issues they are facing. They are eager to hear from your communities!

Your feedback will help local hospitals with their planning and is part of the 2025 Regional Community Needs Assessment.

Participants will receive a $20 Visa gift card and service hours will be awarded.

This event is open for youth ages 11-26 who live in Montgomery County. If you are under 18, your parent/guardian must fill out a consent form. Find the form here (Health survey for youth - consent form) and email it to Sara Huff or bring it with you to the event.
